Volunteer Peer-Tutoring Program Permission Form
%child_first_name% %child_last_name% has expressed an interest in being a volunteer tutor of the school’s peer-tutoring program. This initiative will match responsibility as student tutors with other student’s who are needing extra academic support. Research has shown that peer tutoring has positive impacts on student learning, motivation, and socialization. It can have many benefits for both the tutors and tutees. Volunteer tutors will also receive Community Service Hours for their work which will count towards their high school diploma requirement.
The peer tutoring can take place before school, at lunch, or after school, depending on the schedules of the students involved in the program. The sessions will take place in the school’s library. A contract and log sheet will be given to tutors/tutees so that they are able to contact each other and record the time spent together. A library staff member will sign to verify the tutoring time. Please sign and return the slip below to indicate that you are aware of your daughter/son’s participation in the peer tutoring program. PARENTAL CONSENT AND AUTHORIZATION
I fully understand that upon completing this form, I adhere to the guidelines and policy of the volunteer tutoring program. Moreover, I am aware that %child_first_name% %child_last_name% will be participating in the peer-tutoring program as described above. For this reason, I hereby PERMIT %child_first_name% %child_last_name% to participate in this program and become one of its volunteer tutors.
