Dear Parents/Guardians,

We are very glad to inform you that your son/daughter has expressed his/her desire to become one of our youth volunteers in our Children’s Ministry. Our youth volunteers are reliable youth who commit their time to do some tasks in order for our Children’s Ministry to be successful. However, we have always made it sure for our volunteers to complete this form with their parents so that they can begin to serve as volunteers in our church. By completing this form, you are agreeing to the Church Volunteer Guidelines attached in this document. Also, make sure that your child will personally choose the specific area and time he/she will be available.

PARENTAL CONSENT AND AUTHORIZATION

TAKE NOTE: Youth Workers are required to bring a copy of his/her report card every reporting period.
