Parental Permission Form for Group Tutoring Session in Group Tutoring Room
I give %child_first_name% %child_last_name% permission to receive tutoring from the school’s Group Tutoring Programs for this semester.
The following items must also be checked by the parent in order for the tutoring session to take place:
I understand that the area where the tutorial will happen includes cameras that record any and all activities in the Group Tutoring and those recordings will be kept for 30 days unless there is an incident or questionable behavior within that time frame.I have read and I understand the school's Group Tutoring Policy.I have discussed with my child Group Tutoring Policy of the school, and the use of cameras to video record activities in the Group Tutoring sessions.I fully release, discharge, hold harmless, and defend the school, its trustees and staff, its officers, agents, and employees from any and all claims from injuries, including death, damages, or loss, which may arise or which may be alleged to have arisen out of, or in connection with, any tutoring session.I understand that a new permission form must be on file with the School upon the start of a new school year in which my child is to be tutored in the School’s Group Tutoring Programs, or the tutoring session/s cannot take place.
